The Inc. 5000 list is an annual ranking of the fastest-growing private companies in the United States, covering all industries. Established in 1982, it is highly authoritative. Household names such as Microsoft, Facebook, and Oracle have previously graced the list.
In August 2024, the 2024 Inc. 5000 list was launched, ranking companies based on their revenue growth percentage from 2020 to 2023. Companies must have revenue of at least $100,000 in 2020 and at least $2 million in 2023.

2019年,Monogram Health 成立;2023年1月,获得C轮3.75亿美元融资,至今已累计融资5.47亿美元。
Monogram Health 面向患有多种慢性疾病(尤其是包括慢性肾病和终末期肾病)的患者,提供基于证据的综合诊断、药物治疗、社会支持、跨学科护理协调等居家护理服务。

Vytalize Health provides home and remote care for individuals aged 65 and older, including annual health checkups, medical proxy designations and living wills, chronic disease management, mental health support, and personal care. Working with external providers, care is coordinated by physicians, ensuring they are empowered to represent their patients.
Vytalize Health acts as a bridge between payers and care providers, offering comprehensive services and technical support, including a team of clinical experts, data, technology, referrals, and reimbursement. This helps strengthen care coordination, improve patient outcomes, and ultimately reduce healthcare costs and mitigate payer risk.
Partner institutions have seen an average 41% reduction in emergency department utilization, a 38% reduction in hospitalizations, a 9% reduction in medical costs, and a 17% increase in patient retention. Each institution has received $125,000 in federal government incentives, saving an average of $1,434 per Medicare beneficiary.

In addition to providing these technologies and services, Devoted Health, a similar company, also offers Medicare Advantage, a commercial insurance program for seniors, as a funding source. Founded in 2017, it has raised $2.3 billion in funding and is valued at $13 billion.
2. Home Kidney Care: 438-fold Growth, $547 Million in Funding
Monogram Health ranked third among all industries in the 2024 Inc. 5000, with revenue of nearly $2.2 billion in 2023, a 438-fold increase in three years.
Monogram Health was founded in 2019 and secured $375 million in Series C funding in January 2023, bringing its total funding to $547 million to date.
Monogram Health provides comprehensive, evidence-based home care services for patients with multiple chronic conditions, particularly chronic kidney disease and end-stage renal disease, including comprehensive diagnosis, medication, social support, and interdisciplinary care coordination.
Chronic kidney disease affects 37 million people in the United States. This condition results in an inability to properly filter blood, resulting in elevated waste and fluid levels. Managing chronic kidney disease is complex and can easily lead to poor health.
Traditional care typically focuses on dialysis for late-stage kidney disease. Most people struggle to detect their kidney disease in its early stages, and are unaware of its progression rate. Competitor Somatus, emphasizing early detection and prevention, secured $325 million in Series E funding in 2022.
III. Home Care for Complex Diseases: 307-fold Growth
Upward Health, ranked 6th overall in the 2024 Inc. 5000, saw revenue increase 307-fold in three years. Founded in 2017, the amount of its Series B funding is unknown.
Upward Health provides home care for the most frail patients with complex diseases, fully covered by commercial insurance, Medicare, and Medicaid.
Direct competitor Vesta Healthcare, ranked 448th overall in the 2024 Inc. 5000, saw revenue increase tenfold in three years. Founded in 2018, it secured $65 million in Series C funding in September 2024, bringing its total funding to $160 million to date.
Complex medical care patients represent the top 5% of US healthcare spending, yet they account for 50% of total health care expenditures. This group is primarily elderly, often suffering from multiple chronic conditions, serious illnesses, and behavioral health issues (such as mental health, substance abuse, and intellectual disability).
From the beginning of 2022 to September 2024, at least 13 related financing events were reported in the US, totaling $732 million.
US healthcare providers are fragmented and siloed, and primary care physicians often don't know which specialist to refer patients with multiple medical conditions to.
Complex care providers typically provide medical care, assistance with daily activities, behavioral health care, social support (such as housing, food, transportation, and education), and remote monitoring.
Interdisciplinary teams coordinate care, collaborating with patients' primary care physicians, other care providers, and health insurance plans to avoid unnecessary, duplicative, and conflicting treatments.

Employee recruitment and retention encompasses the entire process from job posting to employee hiring. Aaniie manages all recruitment activities. Through gamification, caregivers are rewarded and thanked for completing tasks, fostering engagement and a sense of accomplishment. The average home care provider in the United States has 50 caregivers, with annual turnover ranging from 65% to over 80%. Podimetrics has developed an easy-to-use at-home mat that patients step on for 20 seconds daily to scan and monitor foot temperature fluctuations. A temperature above normal indicates possible infection; a temperature below normal indicates decreased circulation and an increased risk of diabetic foot ulcers.
Through remote monitoring, doctors can detect foot inflammation early and take preventative measures quickly.
34 million people in the United States, representing 10% of the population, have diabetes, and their lifetime risk of developing a diabetic foot ulcer is 34%.
Diabetic foot ulcers increase the risk of amputation. Because diabetics lack insulin secretion or impaired cellular responses, blood sugar levels spike, leading to symptoms such as decreased sensation in the limbs, impaired circulation, and decreased vision.
As a result, patients may struggle to detect foot injuries and pain, and impaired circulation can delay wound healing. Some open wounds can easily develop into ulcers, which can quickly spread infection and lead to serious complications, increasing the risk of amputation.
Diabetic ulcer amputations cost billions of dollars annually and result in millions of limb losses, with the total cost of lower limb complications exceeding $100 billion. A single amputation can cost up to $100,000, and 62% of patients who undergo an amputation die within five years.

Loneliness is associated with a 45% increased mortality rate and a 68% increased risk of hospitalization, as well as health issues such as obesity and depression. Seniors are reluctant to use technology and seek help, so staff proactively provide assistance by phone.
60% of Pyx Health members have used the app for at least 30 days, 80% reported feeling better after two calls, 67% felt more connected to their healthcare team, and 61% saved on emergency room and hospitalization expenses.

Honor is the first company in the United States to provide technology solutions, operational support, and caregiver support to home care providers. Partners focus on finding and maintaining clients, while Honor handles all other tasks, including caregiver recruitment and training, scheduling, performance management, billing, and invoice management.
Federal government services only cover certain types of care, have time limits, and do not support marketplace payments, making them incapable of meeting all the needs of seniors in their homes. Honor primarily serves large, high-income, middle-class families, providing short-shift, non-medical care services, all at the client's own expense, with a 99.9% shift completion rate.
The shortage of caregivers is a serious industry issue. Honor's unique approach is to improve recruitment and retention by prioritizing caregivers. Over 90% of caregivers are willing to recommend Honor to their friends.
